Case Management for Homebound Seniors

This program provides case management for seniors, so that they can remain safely in their homes. Services include assessment, creation of comprehensive care plans, benefits/entitlements counseling, linkage to home-delivered meals and home care, counseling, information and referrals, and caregiver support. The program addresses all issues associated with advanced age.
