Residential and Housing Services

Our Integrated Health and Wellness Services are as varied as the issues that arise for those having difficulty adjusting at home, school, or in the community – issues such as depression, substance abuse, marital problems and other stress-related conditions.

Among the services provided by professional, dedicated staff are short-term and long-term counseling, stress management, activities therapy, vocational support and MICA services. Our community-based mental health clinics offer those with severe mental illness a broad range of outpatient services, while our residential & housing services provide residents with independent living skills, health management, counselor visits and educational and recreational opportunities. Our care coordination and case management programs help individuals continue living in the community and facilitate access to the services and supports necessary to enhance or maintain their level of functioning and personal satisfaction in their lives. We also focus on prevention, especially for our most vulnerable neighbors: children at risk of being separated from their families. The goals of our prevention programs are to strengthen family relationships in order to reduce the incidence of abuse and neglect.

The Brooklyn and Queens Community Living Programs are scattered-site residential programs that serve 123 seriously mentally ill consumers. The Brooklyn Community Living program provides 51 beds in 18 apartments primarily located in Canarsie, East Flatbush and Old Mill Basin, Brooklyn.  The program apartments are located in residential areas and are convenient to shopping and transportation.  The program’s purpose is to assist individuals who have a psychiatric disability to live as independently as possible within a structure of support services provided by program staff.  The program's mission is to empower residents through the delivery of rehabilitative services, case management, counseling and training in order for them to develop the skills and abilities to live in more independent settings in the community. Staff assists clients with assertiveness/self advocacy, community integration, resources development, daily living skills training, health services, medication management training, parenting training, rehabilitation counseling, skills development services, socialization, substance abuse services and symptom management training.

The program houses and provides services to adults with a severe and persistent mental illness. AXIS I DSM-IV diagnosis is required for a client to be eligible for services. Clients should be eligible for Medicaid and for SSI, SSD or PA.

Central Brooklyn Supported Housing is a program that assist individuals in maintaining independent housing.  The Central Brooklyn Supported Housing program serves 12 clients in apartments located primarily in central Brooklyn in residential areas and convenient to shopping and transportation. The program provides a furnished studio/one bedroom apartment or two bedroom apartments with on-site services which are supported by Transportation Services, Mental Health, Social Services and other generic services within the community.  The staff is available to assist in emergencies and are on-call at all times.  The program’s purpose is to assist individuals who have a psychiatric disability to live as independently as possible with minimal supports which will vary based on each individual's needs.  The mission of the program is to empower residents to remain integrated in their community.  Staff assists clients with vocational goals, social skills, daily living training, budgeting skills, parenting training, medication management and symptom management training.

Individual must be 18 years or older and be diagnosed as Seriously Persistently Mentally Ill and have the desire to live in a supported housing program.
