Responsibilities: 

 

  • To provide a full range of case management and support services for designated residents.

  • Carrying a case load of single cases and family cases.

  • Responsible for all functions necessary to house and support people with HIV/AIDS.

  • Arranges home-health care services.

  • Makes necessary linkages to psychiatric, rehabilitative, medical, substance abuse, legal, & financial services.   

 

Qualifications: 

 

  • Bachelor Degree in a field pertaining to Social Services, Mental Health or related discipline.
  • At least two years experience in the field of Human Services related to the population served, e.g. housing, substance abuse, etc.Knowledge of the special needs for persons with HIV/AIDS.
  • Ability to be mobile concerning apartment visits, assisting clients with appointments and professional liaisons.
  • Bilingual English/Spanish, Drivers’ license and a car is a plus.
