Responsibilities: 

 


  • Assess needs of consumers and prepare service plans.

  • Provide case management, advocacy, group work, and financial assistance to individuals and families to maintain and stabilize housing.

  • Conduct outreach.

  •  Perform home visits.

 

Qualifications: 

 

  • Bachelor’s Degree in Human Services or related field.
  • Computer literate with all Microsoft Office software packages.
  • Knowledge of entitlements helpful.
  • Strong organizational and communication skills.
  • Willingness to travel within five boroughs, mostly within Queens.
  • Willing to work from 11:00 a.m. to 7:00 p.m. on occasion.
  •  Bi-lingual English/Spanish preferred.
