Care and Support in a Nurturing Environment
Catholic Charities serves people with developmental disabilities in Brooklyn and Queens who live at home with their families or caregivers. Among the services we provide to the developmentally disabled are comprehensive case management, home and community services waivers and family support case management. We also provide residences for developmentally disabled adults offering 24 hour supervision, daily skills training, recreation and a variety of psychological, social and clinical services, such as medical care and speech training.
Every day, qualified and competent staff members provide comprehensive social services mixed with warm, friendly emotional support to people with developmental disabilities as well as to their caregivers. At our 21 residences for the developmentally disabled, individuals receive 24-hour supervised care while living independently and experiencing real autonomy.
We also seek to provide people with disabilities meaningful leisure activities which are intellectually and personally stimulating. We place an emphasis on providing an environment in which our clients make their own choices and pursue their own interests, including ceramics, crafts, art, woodworking, movement to music and bowling. Adult education and skill enhancement is provided, as well as weekly community trips to concerts, museums, parks and libraries.

Carmel Residence is a 10-bed facility, that greatly contributes to increased quality of life of persons with Developmental Disabilities and/or Mental Retardation. Carmel Residence provides a warm and supportive home for all the consumers that live there. This nurturing environment provides care to persons of different developmental levels of functioning. Carmel Residence is both an ICF (Intermediate Care Facility and a CR (Community Residence) home. On a daily basis, the team/staff at Carmel Residence ensures that the consumers who we work for enjoy a productive life style. The consumers participate in daily outside day programming, they enjoy and participate in the benefits of their community (going to the neighborhood and wider community - flea market, malls, movies, restaurants, churches, museums, parks, concerts, etc). The consumers who live at Carmel also receive valuable in-house services, for example: a House Manager, Assistant Manager, Registered Nurse, Case Manager (QMRP), Applied Behavioral Psychologist, Nutritionist, Physical Therapist, Speech Therapist, Cook, Housekeeper, Residence Counselors. Carmel Residence's goal is to be the ideal home for those who reside there and to be an example of family living and to keep up with the times, all with service provision and emanence of life. It is Carmel Residence's mission to strive to continue to provide quality care in all aspects of the consumer's life on a daily basis.
In order to reside at Carmel Residence a consumer must require assistance in their adaptability of daily living skills (i.e. self care - showering, dressing, feeding, tooth brushing, hair combing, etc), independent living skills (i.e. managing finances, reading, writing, counting, using the telephone, etc.)

Savio Residence is located in Corona New York, which is a multi-cultured community. We provide holistic and person-centered services to 15 individuals with Developmental Disabilities. The program fosters a growth-producing environment that promotes inclusion, individualization and independence. The home is divided into two programs: an ICF (Intermediate Care Facility) and a CR (Community Residence). The ICF program has 9 individuals and the CR has 6 individuals. Services are provded to the individuals by a team professionals composed of: a Program Manager and Assistant Program Manager who ensure that program is operationally viable; a QMRP who ensures that services are available for the individuals based on their needs; an ABSS who develops plans that addresses behavioral needs of the individuals and also ensure that staff are trained to positively assist the individuals; an RN who ensures that all medical needs of the individuals are met; a dietician who develops menus that meet the nutritional needs of the individuals; and Direct Support Professionals who are on the front lines to implement plans developed by the other members of the team.

Golden Residence is an Intermediate Care Facility which houses 10 consumers functioning within the moderate to profound range of mental retardation. At this residence each consumer is treated as an individual and we strive to address each one of their special needs. We promote as much independence as possible. Graci Residence houses eight individuals. These consumers go to various day programs, and all participate in community inclusions. All consumers go out in the community for walks; they also go to grocery stores, movies, museums concerts and participate in other activities in and outside the community. In the summer all the consumers enjoy barbecues, playing basketball and listening to music in the backyard.
Potential clients have to be able to ambulate without restraint, perform ADL skills and participate in community inclusion and in-house chores.

The Helen Owen Carey Residence is a dual program housing both ICF and CR individuals. The Intermediate Care Facility (ICF) offers a residential treatment option in the community for individuals with specific medical and/or behavioral needs. The ICF also provides 24-hour on-site assistance and training for the individuals, intensive clinical and direct-care services, supervised activities and a variety of therapies. The ICF is designed for individuals whose disabilities severely limit their ability to live independently, while the community residence fosters supportive interpersonal relationships, offers supervision to ensure health and safety and assists in learning activities that are a part of daily living. The community residence portion of the program is designed to provide a home environment, and also to provide a setting where individuals with developmental disabilities can acquire the skills necessary to live as independently as possible.

Home and Community Based Services provides Medicaid Service Coordination, Community Habilitation and Family Support Services to individuals who are diagnosed with MR/DD. MSC provides case management to individuals and family for the purpose of accessing services they are entitled to. James Fitzpatrick is a Individualized Residential Alternative facility. There are eight (8) individuals who resides at this home, six (6) males and two (2) females. These individuals are dually diagnosed with development disabilities and psychiatric disorders. The individual's age ranges from 25-61. Six (6) of the individuals are verbal and two (2) non-verbal, however, they utilize minimal sign language to communicate. Peter J. Connolly is a four story brownstone house. It houses 7 consumers with developmental disabilities (3 adult males and 4 adult females). Our goal is to provide a safe environment through quality care and to help the individuals become as independent as possible. To achieve these goals the following services are provided: a part-time QMRP that creates the valued outcomes, monthly progress notes and plan of protective oversight; a part-time ABSS, who addresses behaviors; a part-time nurse who addresses all medical aspects of their lives and is on call 24 hours; a part-time nutritionist who addresses individual diets and creates the menus. There are three staff on each shift (overnight, morning and evening) who carry out the valued outcomes, conduct fire drills and integrate the consumers in the community. All these staff are trained and certified in the following: CPR, first aid, AMAP, SCIP, OSHA, Part 624 & 632 Regulations.

Here at Straus, what we seek to do is create among the staff a community of caring and sharing made up of people who seek to reach out to the poor, sick, aged, isolated handicapped and alienated. Winter F.U.N. is a Saturday-only recreation program for children and adults with developmental disabilities. It provides socialization opportunities and skill development in interpersonal and activity areas. Participants are engaged in group sports and games, arts and crafts and music. They attend monthly trips, community outings and various educational and interesting destinations. The program provides valuable respite for family care givers, while providing a safe and stimulating environment for those attending. The program also provides volunteer opportunities for high school and college students interested in pursuing community service experience.
Individuals must be ambulatory, able to respond to verbal directions and manage personal care(toiletting) independently. They must be diagnosed with a developmental disability, as established in a psychological evaluation. They cannot exhibit behaviors that are a danger to themselves or others. They must reside within the communities of Brooklyn or Queens.
