Services for Persons with Developmental Disabilities

Through Catholic Charities’ services for People with Developmental Disabilities, compassionate staff members provide care and support to individuals with special needs. For adults with developmental disabilities – and their caregivers – the agency provides comprehensive case management and emotional support. Programs provide clinical, psychological and recreational services and daily skills training in a supportive environment, and our residences for the developmentally disabled provide effective support which helps residents continue to excel in their daily lives.

The Family Support Services Program assists individuals and their families coping with the hardships associated with Intellectual and developmental disabilities access needed services and supports required, to meet the challenges of everyday living.Families receive case management services enabling them to gain a variety of supports and services. These services include OPWDD eligibility, Medicaid, SSI, and food stamps.

The Home and Community Based Community Habilitation Program provides services a to individuals in their homes. Community Habilitation services are designed to assist individuals in acquiring, retaining and improving the self-help, socialization and adaptive skills necessary to reside successfully at home and in community based settings. Community Habilitation Services teach individuals life skills to succeed in attaining their goals. Services can also be provided in licensed OPWDD residences

The Medicaid Service Coordination Program (MSC),  is a Medicaid State Plan service provided by OPWDD which assists eligible persons with developmental disabilities in gaining access to necessary supports and services appropriate to the needs of the individual. MSC is provided by qualified service coordinators and uses a person centered planning approach in developing, implementing and maintaining an Individualized Service Plan (ISP).

MSC is designed to help people strive for the highest quality of life. The focus of MSC is on assisting each person to achieve his or her unique goal and desire relative to the person’s informed choices including but not limited to: choice of home, meaningful work and/or community activities, social and leisure activities; meaningful relationships; and access to services/supports that promote optimal health.
