Catholic Charities Brooklyn & Queens Jamaica HomeBase program
in partnership with the NYC Department of Homeless Services provides
assistance in Community District 12/Queens CD 12 to prevent
you and your family from becoming homeless.
Jamaica
HomeBase is part of Uniting for Solutions Beyond Shelter, New
York City's five-year action plan to end chronic homelessness.
Jamaica HomeBase Homeless Prevention Community
Resource Center is a neighborhood-based program in Community District
12 (Jamaica, Hollis, St. Albans, Springfield Gardens North) providing
assistance to address problems causing homelessness by helping
families and individuals stabilize their current housing. Case
management services are available to address individual needs
such as emergency assistance, advocacy, supportive counseling
and referrals.
Initial Eligibility Criteria:
Families or single adults are eligible for our services if they:
- Reside in Queens Community District 12 or are returning to
a residence in CD 12 from an alternate living, supportive living,
or institutional environment, i.e. imprisonment, hospitalization,
foster care.
- Have a household income that does not exceed 200% of the Federal
Poverty Line.
- Are experiencing a threat to their housing stability that
may put them at risk of entering a shelter, such as *Overcrowded
or doubled-up housing*Family discord or domestic violence*An
eviction proceeding*Hazardous living conditions*Other destabilizing
factors*
