Services offered to the Homebound Elderly

Services for the homebound elderly (Case Management Programs) provide an array of services to people 60 years and older who are homebound. These Case Management Programs have social workers who will go to the homes of the elderly to assess their needs and link them to services that help them them stay safely in their homes and communities for as long as feasible. Some of the services these programs provide and provide linkage to are: case management; Meals on Wheels; homecare and housekeeping for a maximum of 20 hours per week; friendly visiting; in home counseling; as well as transportation to local doctors or neighboring senior centers. These Case Management Programs are opened Monday through Friday, and are funded by NYC Department for the Aging.

Catholic Charities Neighborhood Services Case Management for the Homebound Elderly

Provides friendly visiting and recreational services to homebound seniors in the area of the South West Queens, Monday through Thursday. This program is funded by the Queens Borough President’s Office.

This program provides friendly visiting services for homebound elderly in Community Districts 8, 11 and N 13. These areas are Bayside, Flushing, Hollis, Little Neck, Hillcrest, Jamaica Estates, and Pomonok in Queens.
This program is funded by CITYMEALS ON WHEELS, Inc.

Eligibility. The Case Management programs provide services for people 60 years and over who are homebound who reside in the area served by these programs.

Application: The Case Management Program will provide a social worker to visit the senior in his/her home to complete a free assessment form that is necessary to obtain services.
