With a team approach, the Saint Vincent’s Behavioral Health Home Care works with the patient, physician and family to provide a systematic psychiatric assessment of the patient's behavioral health and to create an individualized treatment plan tailored to meet their specific emotional and medical care needs. Services
provided: Treatment interventions include: Eligibility criteria: Admission to the program is usually completed within 24 hours of receipt of a physician’s orders and the patient’s approval. Referrals are accepted 24 hours a day, 7 days a week. With notice, informational visits and patient assessments can be done on-site. Department at Catholic Home Care is a not-for-profit non-denominational, certified home health agency accredited by the Joint Commission for Accreditation of Health Care Organizations, and a member of Catholic Health Services of Long Island. Catholic Home Care (formerly Nursing Sisters Home Care) has provided skilled professional home health care services throughout Nassau, Suffolk and Brooklyn for almost a century. Patients who need home health services are in the caring and capable hands of New York state registered and licensed practical nurses, physical therapist, speech language pathologist, nutritional counselors and medical social workers. Highly trained and closely supervised certified home health aides are an integral part of Catholic Home Care. Each patient's individualized plan of care is designed by the health care team in conjunction with the patient, family and physician. Our services are accessible through Medicare, Medicaid and most private insurances. A sliding scale fee is available and services are provided on the basis of need, rather than the ability to pay.
