Home care provides skilled nursing and rehabilitative services to thousands of New Yorkers in every stage of life and where they feel most comfortable — in their own homes. All our home care programs are licensed by the New York State Department of Health; accredited by the Joint Commission on Accreditation of Healthcare Organizations; and approved for participation in the Medicare and Medicaid Programs.

Care at Home

Care at Home, Diocese of Brooklyn, Inc. is an affiliate of Catholic Charities which provide at-home care to over 2,500 elderly and disabled clients each year.

Services: assistance with activities of daily Living; meal planning and preparation; escort, shopping and errand services; laundry (in home or at local Laundromat); light housekeeping to maintain a clean & neat environment; respite services to relieve caregiver and allow “time off”, Companionship and emotional stimulation 24 hour availability; home safety supervision for persons with Dementia Nursing Services

Eligibility: In need of personal care or assistance with activities of daily living. Services available for all age groups.

Saint Vincent’s – Long Term Home Health Care


One of the first programs of its kind in New York City, our Nursing Home at Home Program makes it possible for aging or chronically sick people, who might otherwise have no choice but to enter a nursing home, to remain at home in the communities they have known all their lives. Our program offers comprehensive care, including:
• Skilled nursing
• Personal care aides
• Physical, occupational and speech therapy
• Nutrition counseling, including meals
• Social work
• Respite care
• Social day care
• Medication supervision and medical supplies
• Other services, including an emergency alert system.

Telehealth Home Monitoring: If you are living with a chronic condition like diabetes, congestive heart problems, lung and breathing difficulties, or some other complex illness, the new Telehealth system might be ideal. It’s easy to use and allows your Saint Vincent’s doctor, nurse and home care team to monitor your vital signs and symptoms every day, between personal visits. Ask us to help you decide if Telehealth is right for you!

Eligibility: Long term home health care is designed for people who are:
• Chronically ill or disabled, over 18 years of age
• In need of monitoring by a registered nurse
• Under the supervision of a private doctor or clinic physician
• In need of personal care and help with day-to-day activities
• Covered by Medicaid or eligible for Medicaid coverage
• Living in Manhattan, Bronx, Brooklyn, Queens, or Nassau County.

Saint Vincent’s Certified Home Health Agency


Saint Vincent’s Certified Home Health Agency is one of the area’s most experienced home healthcare providers. We offer:

• Skilled nursing care, including home infusion therapy
• Physical, occupational and speech therapy
• Home health aides
• Social work
• Nutrition guidance
• Oxygen therapy.

We work with independently accredited, licensed vendors to provide you with needed durable medical equipment, diagnostic testing and laboratory services.

Special Programs: We offer specialized clinical programs to help you care for yourself, prevent the recurrence of disease and promote wellness. And Telehealth is available for remote monitoring of your condition between home visits. Our specialized clinical programs include:
• Asthma and COPD
• Behavioral health services
• Cardiac care
• Diabetes care
• Infusion therapy
• Rehabilitation services
• Skin and wound care

In addition to clinical services, the Saint Vincent’s Staten Island Early Intervention Program provides behavioral and developmental interventions for children from birth to three years old who experience developmental delays.

St. Vincent's Behavioral Health Home Care

With a team approach, the Saint Vincent’s Behavioral Health Home Care works with the patient, physician and family to provide a systematic psychiatric assessment of the patient's behavioral health and to create an individualized treatment plan tailored to meet their specific emotional and medical care needs.

Services provided: Treatment interventions include:
• Routine mental status assessments
• Routine suicide risk assessments
• Individual and family supportive psychotherapies
• CBT / DBT and behavioral skills training
• Medication management and education
• Individual and family psycho-education
• Routine physical assessment and treatment
• Links to supportive community services
• Home Health Aide education and supervision.

Eligibility criteria: Admission to the program is usually completed within 24 hours of receipt of a physician’s orders and the patient’s approval. Referrals are accepted 24 hours a day, 7 days a week. With notice, informational visits and patient assessments can be done on-site.

Catholic Home Care

Catholic Home Care is a not-for-profit non-denominational, certified home health agency accredited by the Joint Commission for Accreditation of Health Care Organizations, and a member of Catholic Health Services of Long Island. Catholic Home Care (formerly Nursing Sisters Home Care) has provided skilled professional home health care services throughout Nassau, Suffolk and Brooklyn for almost a century.

Patients who need home health services are in the caring and capable hands of New York state registered and licensed practical nurses, physical therapist, speech language pathologist, nutritional counselors and medical social workers. Highly trained and closely supervised certified home health aides are an integral part of Catholic Home Care. Each patient's individualized plan of care is designed by the health care team in conjunction with the patient, family and physician.

Our services are accessible through Medicare, Medicaid and most private insurances. A sliding scale fee is available and services are provided on the basis of need, rather than the ability to pay.
