RESPONSIBILITIES:
- Provide case management services to the mentally ill population.
- Responsible for face-to-face off site contacts, provide crisis intervention, coordination of services.
- Maintain client charts in audit readiness; write daily notes, treatment plans.
- Provide coverage and on call 24 hours for emergency services. Advocate and link clients to services.
QUALIFICATIONS:
- Bachelor Degree in Social Work or related field and 2-4 years experience in working with population.
