RESPONSIBILITIES:
- Provide direct residential, rehabilitative, recovery, clinical and case management services to clients and maintain accurate recording in case files.
- Prepare contact notes, incident reports.
- Participate in the on-call duty roster for 24 hour coverage.
- Make referrals to appropriate community service providers.
- Assist clients with compliance with house rules, with activities of daily living.
- Assist residents in formulating and implementing service plan goals.
QUALIFICATIONS:
BA or BS degree in Human Services field.
Experience working with severe and persistent mentally ill individuals preferred.
Ability to communicate and to work with diverse populations.
