RESPONSIBILITIES:
• Provide face to face individual contact, observations,
assessment of health and social needs
• Integrate inter-disciplinary team (IDT) data
• Act as a co facilitator and coordinator of the (IDT)
• Provide advocacy and linkage to services
• Provide critical analysis of the individuals needs
• Review plan of care for progress and make necessary
revisions
• Schedule and convene annual,semi-annual,and quarterly
review meetings
• Encourage the participation of the individuals in the
development of their own service plan
• Prepare the Comprehensive Functional Assessment
• Prepare and update the CFA at least annually for all
individuals
• Coordinate the development and monitoring of all written
goals and objectives related to the individuals health, safety,
social, and learning needs
• Convenes special clinical meetings
• Responsible to provide crisis management, to assist
in fostering identity, and to ensure service plan compliance
with agency, state, and regulation
• Assist the PM in fostering and maintaining teamwork
approach to service delivery in the assigned home
• Assist Quality Assurance in the investigation of Administrative
reviews, Allegations of abuse, and Serious Reportable Incidents
as assigned by the Project Director
• Required working flexible hours including evenings,
at least two early mornings (6:00 am), and two weekend shifts
per month
QUALIFICATIONS:
• Minimum Bachelor’s Degree with five years experience
working with people within the Services for People with D.D.
field. Masters degree preferred
• Excellent oral and written communications skills.
• Proficiency in computer skills required.
• Ability to train staff
