TITLE: ASSISTANT MANAGER

RESPONSIBILITIES:
- Responsibility for the Family Support Services Program for individuals with developmental disabilities in Brooklyn and Queens.
- Responsibility for intake and initial assessment of individuals from the Brooklyn and Queens communities, seeking services from OMRDD funding sources , entitlement programs (Medicaid, SSI, SSD, Food Stamps) and community resources.
- Provides case management services for eligible persons as they are aided in establishing eligibility for Medicaid Service Coordination, Day Services, other Waiver Services and, vocational opportunities.
- Visits individuals within their homes, residences and day programs on an as needed basis and establishes service agreements with individuals and their families or advocates.
- Serves on committees for Family Support Services and Willowbrook Class Client Services.
- Additional responsibilities include outreach activities throughout the communities and maintaining record books for all individuals served within the program.
- Generates reports on program participation and activity on a monthly and Quarterly basis.
QUALIFICATIONS:
- B.A. Degree in Human Services Area with a minimum of two years experience working with SPWDD population in case management services.
- Masters Degree preferred.
- Excellent oral and written communication skills.
- Ability to work evenings and weekends, as needed.
- Ability to travel throughout the boroughs of Brooklyn and Queens.
- Fluency in English and Spanish preferred.
