TITLE: CASE MANAGER

RESPONSIBILITIES:
- Ability to work in cooperation with staff, volunteers, interns in the execution of assigned tasks including but not exclusive of case service coordination that incorporates compilation of demographic data, screening, needs assessment, service / referral planning, Integrated Service Delivery or community linkage, follow-up regarding service plan goals and objectives, and case closure.
- Provide short-term and crisis counseling to individuals, families and couples as needed.
- Develop internal and external linkages while engaging in community building activities and promoting program services through outreach initiatives.
- Work in collaboration with Community Project Directors and CC staff in implementing and facilitating programs in response to emerging community needs, on and off site.
- Participate in team program planning, staff meetings, case reviews and group services.
QUALIFICATIONS:
- Bachelors degree in Social Work, Human Services, or related field.
- Ability to travel locally and work some evenings / weekends.
- Bi-lingual preferred.
