TITLE: SOCIAL SERVICE COORDINATOR

RESPONSIBILITIES:
- Functioning as a member of a team that provides a holistic integrated delivery of services to the tenants.
- Providing general case management and referrals.
- Serving as a liaison to service providers in the community and monitoring the on-going provision of services from community agencies.
- Providing educational and /or wellness programs for the tenants; submitting necessary reports (HUD, Agency, etc.) in a timely fashion.
QUALIFICATIONS:
- Bachelor’s degree in Human Service preferred.
- Excellent organizational and communication skill.
- Ability to engage people from a variety of ethnic and cultural backgrounds.
- Willingness and ability to work with others as a member of a team.
- Willingness and ability to accompany residents seeking entitlements.
- Prior experience working with the elderly is preferred.
- Fluency in English and Chinese would be helpful.
