TITLE: COORDINATOR

PROGRAM NAME: HOMEBASE HOMELESS PREVENTION

RESPONSIBILITIES:
- Reviews requests for financial assistance.
- Reviews case files for proper documentation for program eligibility at intake and quarterly.
- Reviews and monitors database quality assurance reports.
- Prepares the program for meeting standards of the DHS Performance Stat.
- Assists the program in meeting the requirement of CQI and COA standards.
QUALIFICATIONS:
- MSW or Master’s degree in related field or BA degree with three or more year’s experience.
- Excellent oral & written communication skills.
- Ability to use Microsoft programs (Word, Excel, Outlook.)
- Experience in program monitoring helpful.
